HADM 8140 - Population Health Management Credit Hours: (3) Description This graduate course focuses on critical concepts in population health and population health management. Attention is given to both health care and public health dimensions of identifying, managing, and improving population health outcomes in an era of health care reform. Population-level determinants of health are addressed in detail, as are the rationale and tools for monitoring and improving health problems at a population level. Emphasis is given to specific models for population health management, as well as systemic innovations in health care and public health.
